1. That I am a member of Patra Adhyapak Sangh (Regd. No. 70).
2. That I have qualified the STET/HTET in 1-5th/6-8th/C&V/Lect. Category.(Copy enclosed).
3. That I am qualified for the Post of ........................ in 1-5th/6-8th/C&V/Lecturer Category as per existing service rules of the said cadre.
4. That I am offering my voluntarily service to teach in Govt. School situated in my Block/Distt. If the service of back door entrants Guest Teachers will be terminated as per Hon’ble High Court decision in CWP No.6090 of 2010.
5. That I am offering my voluntarily service to teach in Govt. School situated in my Block/Distt. till the Reguler appointment of Teachers.
6. That I will not claim any benefit regarding my voluntarily service in Govt. School like Pay, TA-DA etc.
7. That I will carry out the duties of teacher honestly & Impartially.
